Can anyone tell me what this is and what to do about it please. He seems fine in all other regards except for that on his comb

It looks like fowl pox (dry). It is generally transmitted by mosquitos. There is 2 forms - wet and dry. The wet form can be serious. The dry form generally runs a cycle and once recovered the bird is immune to further bouts. Basic care is to keep it clean and apply Betadine to help prevent secondry infection.
The other possibility is a recovering injury but the other smaller black spots indicate pox.
