My old Isa Brown is listless , breathing rapidly, off her food and has yellow diarrhoea running out. I hand fed her a couple of worms. She managed to eat them . Has anyone any ideas of what it could be? My other chickens are robust and healthy .Im worried that she could infect the others.

rudi, I wouldn't like to risk a guess as she sounds quite unwell and should probably see a vet if that is an option.
In any case I would isolate her and give her TLC - keep her warm, try to encourage her to eat and drink but don't stress her by forcing her.
